Private equity firm Clayton, Dubilier & Rice and Elevance Health have debuted Mosaic Health, a new national care delivery platform.
Mosaic Health combines the strengths of apree health and Millennium Physician Group. That CD&R and Elevance Health would partner on primary care was announced in April, and the two said Mosaic will offer "a unique set of clinical and digital capabilities to diverse patient populations across the United States."
Mosaic's platform will make it easier for the operating companies to share resources and information, which will allow them to push further toward a whole-person approach to care, according to an announcement. Millennium and apree will become business units within Mosaic but will maintain their own brands, operations and locations.
"Mosaic Health will innovate on existing risk-based care delivery models, and I am excited to work closely with its operating companies and Elevance Health to foster collaboration and help Mosaic Health's operating companies better serve our providers and deliver exceptional care and services to more patients and communities," said CD&R Operating Partner Clay Richards, who will serve as Mosaic Health's executive chairman, in the press release.
"Mosaic Health demonstrates CD&R's continued commitment to investing in and growing innovative healthcare companies that increase access to high-quality and affordable healthcare, and we are excited for this new chapter of innovation, expanded access and growth," Richards said.
Combining the powers of apree and Millennium allows Mosaic to offer a community-based care model that's backed by digital options for patient engagement, care coordination and navigation, according to the announcement.
Mosaic will operate across commercial, individual market, Medicaid and Medicare coverage. Across apree and Millennium, it reaches nearly 1 million patients across 19 states.
Elevance Health also plans to join the capabilities of its Carelon Health unit to Mosaic once it secures regulatory approvals, according to the announcement.
